Hospice Care - Team Care Approach

Erwine provides an interdisciplinary team approach consisting of nurses, home health aides, therapists, dietitians, social worker, physician, chaplain, pharmacist and volunteers. This approach assures that every aspect of the patients care is provided for and that the patient is cared for on every level.

Availability

Services are available to patients in private homes, assisted living facilities and extended care nursing facilities.

Erwine Health Care serves Wilkes-Barre / Scranton, Hazleton, the Poconos and surrounding NEPA counties: Luzerne, Lackawanna, Wyoming, Columbia, Monroe, Bradford, Carbon and Susquehanna.

Related Services

Inpatient and respite services are available when care at home is not manageable. Erwine can also provide specially trained Home Health Aides to provide personal care, mobility assistance, light housekeeping, meal preparation, medication pickup and more to help the patient maintain daily living activities.

Accreditation

Erwine Health Care is a Medicare Certified Agency. Erwine is approved by the Pennsylvania Department of Health, Division of Home Health Services, Medicare and Commercial Insurance and is a member of the Pennsylvania Association of Home Health Agencies. Erwine also received Joint Commission Accreditation with Commendation in 1995 and again in 1998, 2001 and 2004. Erwine received CHAP Accreditation in 2006.
